ath3k devices fail to load firmwares on xHCI buses, but work well on
EHCI, this might be a compatibility issue between xHCI and ath3k chips.
As my testing result, those chips will work on xHCI buses again with
this patch.
This workaround is from Qualcomm, they also did some workarounds in
Windows driver.

btmrvl_add_card() function calls kthread_run that might return error
(e.g. if current thread is killed). If one tries to use the error
value as a pointer then invalid memory access oops happens.
Check kthread_run() return value, if it is an error then release resources
correctly.
TEST=boot computer with BT modules enabled. I see the error message that
BT device initialization failed. Now kernel does not crash. Hint: to enable
BT run 'rmmod btmrvl_sdio; modprobe btmrvl_sdio'

The ath3k driver is treating the version information badly when it
comes to loading the right firmware version and comparing that it
actually matches with the hardware.
Initially this showed up as this:

The isochronous endpoints are not valid when the Intel Bluetooth
controller boots up in bootloader mode. So just mark these endpoints
as broken and then they will not be configured.

The interrupt interface for the Intel USB bootloader devices is only
enabled after receiving SetInterface(0, AltSetting=0). When this USB
command is not send, then no HCI events will be received.

Suspend could fail for some platforms because
btusb_suspend==> btusb_stop_traffic ==> usb_kill_anchored_urbs.
When btusb_bulk_complete returns before system suspend and resubmits
an URB, the system cannot enter suspend state.

In this expression: seq = (seq - 1) % 8
seq (u8) is implicitly converted to an int in the arithmetic operation.
So if seq value is 0, operation is ((0 - 1) % 8) => (-1 % 8) => -1.
The new seq value is 0xff which is an invalid ACK value, we expect 0x07.
It leads to frequent dropped ACK and retransmission.
Fix this by using '&' binary operator instead of '%'.

When detecting a non-link packet, h5_reset_rx() frees the Rx skb.
Not returning after that will cause the upcoming h5_rx_payload()
call to dereference a now NULL Rx skb and trigger a kernel oops.

LDISCs shouldn't call tty->ops->write() from within
->write_wakeup().
->write_wakeup() is called with port lock taken and
IRQs disabled, tty->ops->write() will try to acquire
the same port lock and we will deadlock.

Many btusb devices have 2 modes, a hid mode and a bluetooth hci mode. These
devices default to hid mode for BIOS use. This means that after having been
reset they will revert to HID mode, and are no longer usable as a HCI.
Therefor it is a very bad idea to just blindly make reset_resume point to
the regular resume handler. Note that the btusb driver has no clue how to
switch these devices from hid to hci mode, this is done in userspace through
udev rules, so the proper way to deal with this is to not have a reset-resume
handler and instead let the usb-system re-enumerate the device, and re-run
the udev rules.
I must also note, that the commit message for the commit causing this
problem has a very weak motivation for the change:
"Add missing reset_resume dev_pm_ops. Missing reset_resume results in the
following message after power management device test. This change sets
reset_resume to btusb_resume().

Making a change solely to silence a warning while also changing important
behavior (normal resume handling versus re-enumeration) requires a commit
message with a proper explanation why it is safe to do so, which clearly lacks
here, and unsurprisingly it turns out to not be safe to make this change.
Reverting the commit in question fixes bt no longer working on my Dell
E6430 after a suspend/resume, and I believe it likely also fixes the
following bugs:

PTR_ERR() returns a signed long type value which is limited by IS_ERR(),
it must be a negative number whose range is [-MAX_ERRNO, 0).
The bug here returns negative numbers as error codes, then check it by
"if (ret < 0)", but -PTR_ERR() is actually positive. The wrong use here
leads to failure as below, even panic.

Memory allocated by vmalloc (including stack) can not be used for DMA,
i.e. data pointer on usb_control_msg() should not point to stack memory.

There is currently a race condition in the btmrvl_remove_card() which
is causing hangs on suspend for OLPC. When the race occurs,
kthread_stop() never returns.
The problem is that btmrvl_service_main_thread() calls kthread_should_stop()
and then does a fair number of things before restarting the loop and
sleeping.
If the thread gets stopped after kthread_should_stop() is checked, but
before the sleep happens, the thread will go to sleep and won't necessarily
be woken up.
Move the kthread_should_stop() check into a race-free place.

FW does the synchronization of the different modules during init.
It will report different modules, that it is ready at different times.
The fw download 'winner' will be reported fw ready first. Without this
patch, btmrvl was already continuing before the FW told it too. Probably
on behalf of the 'winner' which then never sees FW ready and times out.

If not winner, driver must release the sdio host lock, so the fw
download can progress. While holding the lock fw download is stalled
and the following error is produced:
