x86/mtrr: Replace size_or_mask and size_and_mask with a much easier concept
Replace size_or_mask and size_and_mask with the much easier concept of high reserved bits. While at it, instead of using constants in the MTRR code, use some new [ bp: - Drop mtrr_set_mask() - Unbreak long lines - Move struct mtrr_state_type out of the uapi header as it doesn't belong there.
