Allow local MVD recordings span multiple levels, whose maximum
amount is controlled by `sv_mvd_max_levels' cvar.
Print a developer warning whenever PF_StartSound is called on
entity not currently in use.
MVD_ClearState now clears all edict_t and mvd_player_t fields.
